Persons who have received medical education abroad, submit a document on the recognition of their knowledge and (or) assignment of the qualification "doctor". Upon admission, the original document of higher education and (or) the document of recognition of knowledge, assignment of the qualification "doctor" is submitted to the admissions committee;

copy of the internship completion certificate (if any);

information about the certificate or certificate of a healthcare professional (copies of documents issued by a foreign state);

an identity document or an electronic document from the digital document service;

copy of the employment record (if any);

six 3x4 cm photos and an envelope for storing photos;

medical certificate in the form 075/u approved by the Order of the Acting Minister of Health of the Republic of Kazakhstan dated October 30, 2020 No. KR DSM-175/2020 "On approval of forms of accounting documentation in the field of health care, as well as instructions for filling them out "(registered in the Register of State Registration of Regulatory Legal Acts for No. 21579);

a copy of the appendix to the certificate of completion of the internship (if any);

copy of the higher education document;

a copy of the annex to the higher education document;

cardboard binding (for storing personal file documents).
